Foreclosure  Intervention Application Checklist

 

To assist us in providing you with the most effective and efficient services please complete the entire application package and attach all required documents along with the application and provide the entire package to us. If you have downloaded the application. All requried forms are provided but you must still provide, pay stubs, banks statements and mortgage documents. If any borrower owns all or part of a business, the borrower must provide a profit and loss statement  for the past 12 months.  If the income shown differs significantly from prior years and explantion must be included.

 This checklist is provided in order to show you what items are required in conjunction with your application for assitance in obtqaining a loan modification. Please complete all sections of the application as each section is important and will help us to assist you. Failure to provide this information can result in a delay in processing your application in a timely manner.

  1.  Foreclosure Prevention Application-Completed w/ the signed authorization page (We cannot offer assitance without this infomation)
  2.  Check the Fannie Mae and Freddie Mac  by clicking on these links to see if they own your mortgage.
  3.  Verification of monthly Homeowner Association Dues, Homeowner’s Insurance policy. Send copies of bills or payment coupons if possible
  4.  The first 5 pages of your “Note”- this document  says NOTE on it and a copy should have been given to you when you signed your mortgage.
  5.  Pay stubs-One month of the most recent one month for all household members. If Self-Employed, please provide a current profit and loss statement for the recent quarter as well as all schedules of the tax returns.
  6.  Tax return for the recent year w/all schedules attached including the W-2’s and/or 1099’s. 
  7.  Banks statements for the most  recent two months.
  8.  Social Security, Pension, Child Support/Alimony, Retirement benefits-proof is needed; a recent award letter and proof that it will continue for a minimum of 3 years.
  9. Request for IRS transcript
  10. Hardship Affadavits  - Must be signed by every person on the loan.
