Crossroads Independent Living Program

The Crossroads Independent Living program provides services for mentally ill youth, ages 18-23, who are aging out of the foster care system or other young adults who would otherwise be homeless. The program serves approximately 12 residents at a time and provides structure, support, life coaches, and a Resident Manager on-site to assist the youth with day to day functions.
The youth served By Crossroads suffer from a variety of mental illnesses, from Bipolar Disorder to Schizophrenia, making it difficult for them to transition into adulthood without assistance. Youth in the program have often transitioned from settings such as psychiatric hospitals, therapeutic foster care, and therapeutic group homes. Although the shift from a residential setting to an independent setting can be quite challenging, the youth are assisted by their Life Coach to make the transition with as little difficulty as possible. The program's Life Coaches provide intensive one-on-one and group support, designed to enhance life skills.
Parent-Child Center provides the setting for an environment consistent with an adult living environment. An attractive setting at Sonoma Bay consists of 3 luxury town homes that offer 3 bedrooms and three bathrooms for three to four residents in each home. Included on the property are tennis courts, a fitness center, and a swimming pool. Close to the community are a variety of businesses, including Wal-Mart and Target, where youth can see opportunities for employment. In addition to many businesses, bus stops are within walking distance and allow residents easy access for getting to work and school. Additionally, a Resident Manager lives on-site and provides assistance, guidance, and support for the youth.
Program Services
Crossroads provides an intensive level of assistance for the youth to develop life skills, education, obtain employment, address mental health issues, access psychiatric services, and access community services.
Each resident receives a personal Life Coach that assists and guides them in their journey to lay a solid foundation for their future. Life Coaches are assigned to the youth when they enter the program and assist them in a variety of ways. Life Coaches assist with daily life skills, money management, educational needs, and employment assistance. Life Coaches are available to residents 24 hours a day. The youth are also assisted by the resident manager who lives on-site in order to easily assist the youth with any important issues, such as medication, personal conflicts, and issues in the community.
Less than half of the residents of Crossroads have any type of family support and most have a very limited or nonexistent support system. Crossroads provides them with a family type environment that is based on limits, boundaries, and consistency.
